DESCRIPTION: This application proposes to use a quasi-experimental design to
assess employee impact relative to the delivery of health care to children with
special needs in Indiana. The design will exploit a natural experiment where
ICHIP (Indiana CHIP) will use a Medicaid managed care expansion in Phase I
which will serve as the control condition and will be compared with Phase II of
ICHIP as it moves to a separate State Health Insurance Program. An additional
comparison will be between Risk-Based Managed Care (RBMC) and Primary Care Case
Management (PCCM) within both phases. The following specific aims will be
addressed with both qualitative and quantitative methods: (1) Describe the
structural, organizational and implementation features of Phase I and Phase II
of CHIP that facilitate coordination and collaboration of services for optimal
outcomes of CSHCN; and (2) Evaluate, within and between, program comparisons of
outcomes for CSHCN including: access to care, utilization of services, quality
of care, satisfaction with care, expenditures for care (evidence of "crowd
out"), health outcomes, and family impact. The following comparisons will be
made: pre- and post-enrollment outcomes for CSHCN; CSHCN who receive services
through the managed care model of RBMC to those enrolled in PCCM; and CSHCN
enrolled in a comprehensive package of benefits under EPSDT in Phase I to CSHCN
with a basic service plan and wrap-around services in Phase II.
